Case #5
Clinical data: 53 year-old man with pancreatic tumor. Peritoneal nodule
0.6 cm. Metastasis?
Frozen section diagnosis: Metastasis of adenocarcinoma, Grade 3. Pancreatic
carcinoma as primary tumor is possible.
Pathology and comment: Small metastatic infiltrations of the peritoneum
may be a diagnostic problem. Fibrotic changes and only few tumor cells make the
diagnosis difficult, especially if no primary tumor is known. Diagnosis of
peritoneal metastasis may lead to the completion of the operation. Therefore, diagnosis
of malignancy should be make very carefully, especially if the primary tumor
is locally resectable. A further help may be the removal of other suspect nodules.
DD: s. Case 4, 6, 7.
