Case #12
Clinical data: 50 year old man with carcinoma of the gastric cardia. Is
the resection margin free of tumor?
Frozen section diagnosis: Lymphatic invasion of tumor cells on the resection
surface of the esophagus. PAS reaction.
Pathology and comment: We always recommend the histological examination
of esophageal resection margin if a signet ring cell carcinoma of the cardia is
known or in all esophageal carcinomas independently of the macroscopical distance
to the resection margin. Both tumor can spread in the submucosa or in lymphatics.
This infiltration is macroscopically not or only seldom to recognize.
