Comment: Whipple's disease is a very rare disorder caused by Tropheryma whipplei and affecting more frequently Caucasian men than women. Endoscopically the affected small bowel is rigid with thickened mucosa and white-yellow plaques. Histologically the lamina propria of the duodenum was filled by foamy macrophages containing sickle shaped, PAS-positive bacterial fragments (SPC-cells). Lymphangiectasia and lipid globules were also seen. PCR-test confirmed the histological diagnosis. Long time antibiotic therapy is recommended. Prognosis depends on the relapses and CNS involvement.
