Comment: 57 years-old man, with clinical diagnosis of hydrocele. Histologically a papillary tumour with positivity of AE1/AE3 and Calretinin was seen. Our diagnosis: biphasic mesothelioma.
Mesotheliomas of the paratesticular structures are rare. They occur in the age between 55 and 75. Recurrences may be in the first two years. About 40% of the patients die even in the case of well differentiated tumor.
Microscopically the tumors are identical to the mesotheliomas of the pleura. Immunohistochemistry demonstrates positivity with AE1/AE3 and Calretinin. CEA is usually negative.
Differential diagnostically: benign mesothelial proliferations must be distinguished. Macroscopic polypoid tumor mass in a hydrocele is most probably a mesothelioma while macroscopically unsuspect lesions of the tunica vaginalis represents benign mesothelial proliferations. Other tumors can be differentiated by immunohistochemistry.
